Savannah River Nursing is actively seeking highly motivated and empathetic Home Health RNs and/or LPNs to join our growing team in Grovetown, GA. We are committed to providing exceptional, personalized care that promotes independence and improves the quality of life for our patients.

What You'll Do:

As a Home Health RN or LPN with Savannah River Nursing , you will provide skilled nursing care and support to patients in their residences. Your responsibilities will include, but are not limited to:

• Performing comprehensive patient assessments: Evaluating vital signs, physical condition, and overall health status.

• Developing and implementing individualized care plans: Collaborating with physicians, patients, and their families to ensure holistic and effective care.

• Administering medications and treatments: Including wound care, IV therapy, injections, and catheter care.

• Monitoring and documenting patient progress: Maintaining accurate and timely electronic health records.

• Educating patients and families: Providing instruction on disease management, medication adherence, self-care techniques, and safety in the home.

• Coordinating care with the interdisciplinary team: Communicating effectively with physicians, therapists, social workers, and other healthcare professionals.

• Providing emotional support and companionship: Building strong, trusting relationships with patients and their families.

• Identifying and addressing potential safety concerns within the home environment.

• Assisting with activities of daily living (ADLs) as needed.

What We're Looking For:

• Current and active Georgia RN or LPN license in good standing.
• Graduation from an accredited nursing program (ADN or BSN preferred for RNs; LPN diploma/degree for LPNs).
• Minimum of 1 year of recent clinical experience, preferably in home health, medical-surgical, or critical care, is a plus.
• Strong clinical assessment, critical thinking, and problem-solving skills.
• Excellent communication, interpersonal, and organizational abilities.
• Ability to work independently and manage a diverse caseload.
• Compassionate and empathetic approach to patient care.
• Reliable transportation and a valid driver's license.
• Basic computer proficiency for documentation.
