Job Description

**Responsibilities** **Position Summary:** Under direct supervision the incumbent identifies and evaluates potential clients as total persons and identifies the functional limitations which impede independent living. This position reports to the Supervisor Care Manager **Qualifications** **Minimum Qualifications:** + Two years of experience providing social service in one or more of the fields of aging health and long term care services required + Bachelors degree in social work from an accredited school required (BSW) + Ability to relate to clients and families + Excellent communication and presentation skills **Overview** Dignity Health Connected Living has been serving seniors in Shasta County since 1968.
